Heart failure is the number one admitting diagnosis for patients 65 and over. Managing a chronic disease such as heart failure can be overwhelming to a patient and their family. In the Clinic, patients get the one-on-one attention of an advanced certified nurse practitioner who works with the patient to improve their quality of life and prevent the recurrence of health failure symptoms. The Clinic can be an instrumental tool for patients who are newly diagnosed with heart failure, patients who have been recently hospitalized with heart conditions, patients suffering cardiac side effects from other diseases, and patients who do not respond to contemporary treatment options or recommended drug therapies.
