Floyd County Residents are Asked to Complete a Health Needs Assessment

In 2011 Floyd Memorial Hospital and Health Services began a journey to identify the health needs of Floyd County and input is now needed from Floyd County residents.  Beginning in January through February 15, 2012, Floyd County Residents are encouraged to visit http://floydcountysurvey.questionpro.com to complete a survey about the health needs of the community. 

The goal of this assessment is to identify areas in which Floyd Memorial, along with community partners, can make a positive impact in the overall health of the community.  All responses will remain confidential and secure.  Floyd Memorial has contracted with QuestionPro, an independent research firm, to field your confidential survey responses. 

The findings from this survey, along with focus group data, will be compiled with Floyd Memorial’s Community Health Network Area (CHNA) Report.  The CHNA report is a compilation of health and quality of life indicators for Floyd County.  This is quantitative secondary data based on what has already been reported by various agencies. 

Floyd Memorial is now looking for primary or first-hand data, which is why this community survey is so important.  Again, all responses are confidential.  Floyd Memorial will simply compile the responses to add to their data.  Once all the data is compiled it will be available to view on the Floyd Foundation website by going to www.floydfoundation.org.
